यस केयर फाउंडेशन शुगर के मरीजों की देख भाल नोर और उनकी सहायता करने के लिए संकल्पित है। संस्था के हेल्थ वर्कर घर पर जाकर शुगर के मरीजों को संस्था के साथ जोड़ेंगे और समय समय पर उनका ध्यान रखेंगे । लखनऊ और देवीपाटन मंडल में हेल्थ वर्कर की भर्ती की जा रही है। मेडिकल डिप्लोमा धारक करीब १०० हेल्थ वर्कर की भर्ती की प्रक्रिया शुरू हो चुकी है। ३ महीने के अंदर पूरे प्रदेश में करीब एक हजार हेल्थ वर्कर की भर्ती की जाएगी। हेल्थब्वर्कर के द्वारा सभी शुगर कें मरीजों को संस्था से जोड़ा जाएगा और उनके शुगर लेवल पर नियमित नज़र रखी जाएगी। मात्र पांच रुपए प्रति दिन के खर्चे पर हर महीने ब्लड, युरिन टेस्ट, ब्लड प्रेशर, वजन और खानपान की देख रेख संस्था के द्वारा की जाएगी। पाविका डायग्नोस्टिक सेंटर चांदपुरा चौराहा निकट ट्रामा सेंटर पर प्रत्येक रविवार को फ्री हेल्थ चेकअप कैंप लगाया जाएगा। जिसमें शुगर के मरीजों और गर्भवती महिलाओं को मुफ्त सलाह और छूट पर दवाएं दी जाएंगी। ब्लड शुगर मात्र तीस रुपए में किया जाएगा। कोई भी पैथॉलजी जांच और अल्ट्रासाउंट आधे रेट पे किया जाएगा। सभी पैथोलॉजी की जांच प्रदेश स्तर पर चल रहे बड़े पैथोलॉजी लैब से करायी जाएगी। संस्थान हेल्थ वर्कर के माध्यम से शुगर के मरीजों की देख भाल करने के लिए संकल्पित है, ताकि अनियंत्रित शुगर लेवल से होने वाले गंभीर बीमारी से शुगर के मरीजों को बचाया जा सके। जिसमें आप सभी लोगों का सहयोग अत्यंत महत्वपूर्ण है। सिर्फ उत्तर प्रदेश में करीब साढ़े चार करोड़ लोग शुगर की बीमारी से ग्रसित हैं जिनको शुगर से होने वाले रोगो से बचाने का संकल्प यस केयर फाउंडेशन ने लिया है। जिसमें आप सभी का सहयोग प्रार्थनीय है।

YES Care Foundation is dedicated to supporting, caring for, and improving the lives of people living with diabetes. Our mission is to ensure that diabetic patients receive regular health monitoring and timely medical guidance to help prevent serious complications caused by uncontrolled blood sugar levels. Our trained Health Workers will visit patients at their homes, enroll them as Foundation members, and provide continuous care and support through regular follow-ups. We have already begun the recruitment of approximately 100 qualified Health Workers with medical diploma backgrounds in Lucknow and the Devipatan Division. Within the next three months, we aim to expand our network by recruiting around 1,000 Health Workers across the state of Uttar Pradesh. Through these Health Workers, diabetic patients will receive regular monitoring of their health, including: Monthly Blood Sugar Testing Blood Pressure Monitoring Weight Monitoring Blood & Urine Tests Diet and Lifestyle Guidance All of these services will be provided by the Foundation at an affordable cost of just ₹5 per day. In addition, free health check-up camps will be organized every Sunday at Pavika Diagnostic Centre, Chandpura Chauraha, near Trauma Centre. During these camps: Diabetic patients and pregnant women will receive free medical consultation. Medicines will be available at discounted prices. Blood Sugar Test will be offered for just ₹30. All pathology tests and ultrasound services will be available at 50% discounted rates. All laboratory investigations will be conducted through reputed state-level pathology laboratories to ensure reliable and accurate results.
